 Hi, I am M. Sumanth Reddy (Roll no:64) 5th sem medical student. this is an online elog book to discuss our patients health data after taking their consent.This also reflects my patient centered online learning portfolio. A 30yr old male came to the hospital with complaints of weakness  Chief complaints: The patient came to the hospital on tuesday with chief complaints of weakness and shortness of breath History of presenting illness: The patient was apparently asymptomatic 1yr ago then he visited a local hospital with complaints of weakness and pain at the lumbar region,then dialysis was recommended at the local hospital,for dialysis he visited Gandhi hospital,medications were given 6 months and told to stop alcohol and meat.he took medications for 1 month and and then stopped,he also started consuming alcohol.Then he visited Kims hospital 6 days ago with complaints of shortness of breath. Negative history: Fever- No history of fever Shortness of breath: history of shortness of breath

73yr old male with tremors

A 73yr old male came to the hospital with complaints of tremors Chief complaints: The patient came to the hospital on tuesday with chief complaints of shivering of hands and legs History of presenting illness: The patient was apparently asymptomatic 1yr ago then mild tremors of upper limbs has begun,which then eventually tremors of lower limbs has also begun.Initially the severity of the tremors are mild later the intensity has increased.There has been tremors of upperlimb even while eating.The severity of the tremors increased when the patient was holding any object and decreased while sitting idle. Negative history: Fever- No history of fever Shortness of breath: No history of shortness of breath  Past history: No significant past history  Personal history: Married Occupat

  JANUARY 3RD 3/1/2023 General medice elog Hi , I am M Sumanth Reddy of 3rd semester student. This is an online elog book to discuss our patients health data after taking his consent. This also reflects my patient centered online learning portfolio.  CHEIF COMPLAINTS  A 75year old male who is farmer by occupation  came to OPD with cheif complaints breathlessness and swelling in legs since 1 month. HISTORY OF PRESENT ILLNESSES  Patient was apparently asymptomatic one month ago.Then he had slowly increasing breathlessness,and developed pedal edema. There is history of cough which is productive(sputum). There is no h/o of fever, burning micturition, decreased frequency of micturition, chills and rigor. PAST HISTORY  No history of Diabetes ,BP  No history of Asthma,TB,CAD,Epilepsy  FAMILY HISTORY No history of Diabetes ,BP  PERSONAL HISTORY Mixed Diet  Appetite- normal Sleep adequate Bowel and bladder movements-normal  GENERAL EXAMINATION Patient is conscious,coherent and cooperative moder

Chief complaint  62 year old male patient come with  complaint of nausea, vomiting from last 2 days early fatigue from last 1 week and pedal edema from last 2 months History of present illness A patient is asymptomatic 2 months back then he developed pedal edema and easy fatigability since 10 days .  CHIEF COMPLAINTS:- A 60 year old patient came to the OPD with complaints of pedal edema in both the legs since 2 years and burning micturition.  HISTORY OF PRESENT ILLNESS:- The patient was apparently asymptomatic 2 years ago after which he developed pedal edema on both his legs. The patient also complained of back ache .The patient complained of pain in his legs while walking and difficulty in standing up due to weakness. The patient also complained of breathlessness while walking . The patient complained of burning micturition since 2 months.No history of fever. PAST HISTORY:- The patient is a known diabetic since 10 years. The patient reported of having ear deafness since 6 years. The patient visited a government hospital 5-6 months ago where he was diagnosed with kidney failure and was put on dialysis. The patient was diagnosed with hypertension 3 months ago No H/O of TB, epilepsy , asthma  FAMILY HISTORY:-  No significant family history . PERSONAL HISTORY :- Diet- vegetarian  App

This also reflects my patient centered online learning portfolio.  CHEIF COMPLAINTS  A 55 year old male who is farmer by occupation resident of achampeta came to OPD with cheif complaints of swelling of the right foot and tingling sensation of both right leg and right upper limb since 10 days HOPI  Patient was apparently asymptomatic 10 days ago.Then he had sudden weakness in the right upper limb and right lower limb.Patient was unable to walk and lift his right upper limb.Then he consulted nearby RMP 10 days ago where he was given some medications.Patient had a history of injury to the big toe followed by swelling of the right foot.Still the patient has no relief from the symptoms so he visited Kims on 27 December. PAST HISTORY  No history of Diabetes ,BP since 6 months  No history of Asthma,TB,CAD,Epilepsy  Patient compl

20th Dec 2022   CASE DISCUSSION  A 55 year old male came  to opd  who was a labourer by occupation  came with CHEIF COMPLAINTS: Vomitings ( since 2days ) Pain in abdomen  Breathlessness  Decreased appetite( since 6days ) HISTORY OF PRESENTING ILLNESS: Patient was apparently asymptomatic 6days ago and then he had decreased appetite and was consuming alcohol. vomitings started 2days ago along with breathlessness. PAST HISTORY: No cold No vomiting  No BP No diabetes No asthma No TB No Epilepsy PERSONAL HISTORY: Married Mixed diet Regular bowel movements No allergies  Normal micturation Alcohol-Daily 180 ml since 20 yrs Cigarettes- Daily 10 cigarettes since 15 yrs FAMILY HISTORY: No significant family history GENERAL EXAMINATION: No pallor icterus is seen No cyanosis No lymphadenopathy  No malnutrition  No clubbing of fingers Pedal oedema is seen FEVER CHART: HEMOGRAM: ECG: TREATMENT: 1.
